Letter of Recommendation
Counselor Letter of Recommendation Request Form: Senior Profile (Brag) Sheet
If you will need a letter of recommendation from your school counselor, you must:
1. Complete the attached form.
2. Schedule an appointment with your counselor.
3. Bring the completed form with you to your appointment with your counselor.
4. The completed form must be turned in at least three weeks in advance to ensure your
letter will be completed by the due date.
NOTE: DEADLINES ARE THE STUDENT’S RESPONSIBILITY
